This example is a demo version of a database used in Don Nickel's web-based Counseling 100 class. It uses an online form allowing students to enter data to submit their class schedule and then makes it available on-line for review by counselors. This is a flatfile PERL cgi writing to text files on a linux server. It was built using an on-line wizard at the Flattext site, and then modified to reflect the De Anza paper form.

This example is a simple database with form inputs for entering student information and searching records. Feel free to explore how it works. Start by going to the Data Input page and completing the form with imaginary values.Check the result using the Search page. Be sure to use a SS# and password you will remember, or you will not be able to retrieve your results.

A modified version of this database is being used as part of the on-line COUN100 course at De Anza. It is used by De Anza counseling to view the Ed Plan submissions from the on-line students and provide feedback to them about their proposed schedule. Counselors have a master password that allows them to view and modify any record in the database.
